3. Attach self-attested copies of the required educational documents, experience certificates, and caste certificates.

4. For candidates who must pay the fee, obtain a Demand Draft of Rs. 500/- in favor of 'ESI Fund Account No. 1' payable at Kakinada.

5. Send the hard copy of the application, resume, and documents via registered post or in-person to the Medical Superintendent, ESIC Hospital, Kakinada-533001 by 18.07.2026 (01.00 PM).
