Prepare a single JPG image by scanning a color photo, signature, and two lines of self-declaration text as per the provided link and instructions (photo in upper part, signature in lower part, white background for photo, photo date and name). Attach this image to the application form.

Review the filled application form for correctness before clicking "Submit".

After submission, note down the application number.
