2. Prepare the application form in the prescribed format and obtain a Demand Draft of Rs. 500/- (if applicable) in favour of 'ESI Savings Fund A/c No. 2' payable at New Delhi.

3. Attach two recent passport-size photographs and self-attested copies of relevant certificates/testimonials (DOB proof, Education, Experience, DMC Registration, etc.).

4. Report directly to the Dean Office, 5th Floor, Admn. Block, MS Building, ESIC MC & Hospital, Basaidarapur, Delhi-15 on 23-09-2026 between 09:00 AM and 11:00 AM with the application and original documents.
